Home At Last Program
This unique program provides a safe transfer from Hospital to Home. Our Team assists individuals who are 55 years of age and older on a safe way home. CHATS does provide support for those who are younger than 55 who may struggle with mental health challenges.
One of the most common things our staff and volunteers encounter is the stress of the caregiver and client. The caregiver feels overwhelmed and worries if they cannot support their loved one. The caregiver themselves may be experiencing burnout. They may have been supporting their loved one for a lengthy period and not sure if they can cope. The client we are taking home may feel like a burden and worry about how they will cope with the changes they’re going through.

· “Will I be able to do what I used to?”
· “Will my spouse be okay without me while I recover?”
These are real, valid concerns, and we’re here to ease them.
Our team of Personal Support Workers (PSWs) truly loves what they do. Each day brings a new opportunity to make someone’s life more comfortable, more secure, and more hopeful. Whether it’s helping a client settle back into their home after a hospital stay or simply being there to offer a listening ear, we understand that we’re not just supporting individuals, we’re supporting families.
For those using home care services for the first time, it can feel a little overwhelming. After all, inviting someone new into your home is personal. But rest assured, our team is not only professional, we’re compassionate, respectful, and focused on your comfort and independence.
We’re here to make the transition easier; for both the client and their loved ones, with care that is as skilled as it is human.
