Our mission is to advocate for and deliver high-quality person-centred home and community services to older adults and their caregivers. Discover the seven easy steps you can take to get started with CHATS today!
Clients can grow with CHATS as their needs evolve or change and the level of care required increases.
Changes to the care plan can be requested at any time.
Initial Visit
CHATS staff members will reach out to you, your family members, or a substitute decision-maker (also called an SDM) during your initial visit to a CHATS program.
Intake Assessment
With the information gathered at intake, our CHATS team will work with you or your care team to develop an individualized care plan (also called an ICP), that accommodates your needs and preferences.
Goal Planning
Next, goals are set!
Sign Off
You, your family, or your SDM will sign off on the agreed-upon care plan.
Care Plan is Recorded
The details of your care plan will be recorded in our secure client database.
Care Plan is Shared
Your individualized care plan is shared with all appropriate CHATS staff to ensure you are provided with the best support.
Ongoing Communication
Your plan is reviewed regularly to review goals and the overall plan with you, your family, or SDM.

Respite care is personalized in-home care for clients and their families, allowing caregivers to take a break.
Yes! We have certified Personal Support Workers who can provide in-home care to loved ones when caregivers need a break.
We also host seven Adult Day Programs across our catchment area where older adults can unlock the benefits of exercise, mental stimulation, crafts, relaxation, entertainment, and more.
